To determine Medicaid reimbursement policy targets and delays and to estimate the effect of reimbursement policies on patient access delay, we linked 2011 Medicaid Discharge Abstracts data and a Medicare enrollment file in two states. We used multivariate logistic regression to determine the proportion of Medicaid enrollees with nonpatient access delay. Using a regression discontinuity design, we estimated the effects of Medicaid reimbursement policy changes over the 2003-2011 period. Medicaid reimbursed providers on the basis of per-visit, per-capita, and per-beneficiary fees. We compared cases with prereimbursement nonpatient access delay (access delays in excess of 3 days) with those whose access delays were shorter than 3 days. The rate of nonpatient access delay in our sample was lower in early than in late years of the study period. States where the target for reimbursement shifted from per-visit to per-beneficiary fees had higher rates of nonpatient access delay. A change in Medicaid reimbursement policy may increase nonpatient access delay in states that focus Medicaid resources primarily on beneficiaries. States can reduce access delay by requiring Medicaid reimbursement
