We investigated associations of body mass index (BMI), %body fat and waist circumference with fatigue severity, and explored potential underlying mechanisms. Patients from the Program of Initial Evaluation of RA (PRIOR) cohort completed validated questionnaires on fatigue (Brief Fatigue Inventory), symptoms (PROMIS-29), disability (Disability index of the Rheumatoid Arthritis Impact of Disease, RAID), physical function (Health Assessment Questionnaire, HAQ) and stress (Perceived Stress Scale, PSS) at baseline and at 12 months. Fatigue severity, BMI, waist circumference, and %body fat were obtained at baseline and follow-up, and several measures of inflammation were examined at baseline. A total of 2,310 RA patients with moderate to severe fatigue (baseline median FIQR of 3.7) were included, of whom 15% were obese, 44% overweight, and 39% normal weight. In multivariable models, higher BMI, %body fat, and waist circumference were associated with more severe fatigue (FIQR ≥ 4.8), after adjusting for age, sex, disease activity and medications.
