While the HR in preterm infants changes with gestational age, it is not clear how the HR following DCC compares to the HR in full-term infants receiving DCC of 2 to 4min. A cohort study of infants 34 to 41 weeks gestation (n=180) had similar median HRs (median=115bpm for both groups) throughout the first 10min of life. 46 Comparing the earlier cohorts of preterm infants, infants 34 to 37weeks gestation had significantly lower HRs at birth than infants 38 to 40weeks gestation (105 vs 123bpm, p=0.005), with greater HR increases at 2min of age in the later gestation (105 vs 86 bpm, p=0.005). p53 is a well-recognized tumour suppressor gene that is frequently mutated in many human cancers, but its central role in cancer is the suppression of angiogenesis. Classically, the p53 tumour suppressor gene performs its anti-proliferative function by inducing cell cycle arrest and apoptosis. However, angiogenesis is an important key step in the metastatic cascade and many human tumours are angiogenesis-dependent, which suggests that p53 is involved in the regulation of the angiogenic switch. There is also evidence that p53 is angiogenesis-independent, and it is becoming clear that p53 also regulates a wide array of biological processes that may have important implications for tumour evolution. In this issue, we review the current literature on the effect of p53 on angiogenesis focusing on both intrinsic and extrinsic angiogenic factors. p53 also regulates matrix metalloproteinases (MMPs) that were first identified as key mediators of invasive tumour cell growth. We will also discuss recent data on the different mechanisms by which p53 regulates MMPs. MMPs control the extracellular matrix (ECM) which includes the basement membrane. ECM remodelling facilitates tumour cell invasion and metastasis. Understanding the role of p53 in regulating MMPs may lead to novel therapies for the treatment of cancer.
