Lymphoma is also a cause of a mass in the gastrointestinal tract in young patients. Magnetic resonance (MR) imaging provides the best current non-invasive approach for assessing bowel wall involvement by lymphoma in this patient group. This diagnostic information is also useful for planning definitive treatment. Other body sites, such as liver and salivary glands, are occasionally involved in patients with inflammatory bowel disease and can be the presenting feature of the disease.
In children with known inflammatory bowel disease, MR imaging of the abdomen and pelvis is most useful for detecting and characterizing complications of the disease. MR imaging is also used to evaluate response to treatment, monitor recurrence of disease, and to detect bowel wall complications such as walled-off necrosis, abscess, and fistula formation.
The major abnormalities that can be visualized by MR imaging can be divided into two groups: (a) those caused directly by inflammatory bowel disease, such as mucosal ulceration and edema (edema is a normal finding and does not indicate active disease) (Figure 1), and (b) those caused by the process of IBD, such as strictures, fistulas, abscesses, and diverticular disease (Figure 2). MR imaging is also important for the evaluation of congenital abnormalities such as intestinal duplications and vascular malformations.
