This study aimed to evaluate the clinical and radiological outcomes of resection of posterior fossa ependymoma with unilateral or bilateral decompression. METHODS Seventy patients with ependymoma in the posterior fossa treated at Chinese PLA General Hospital were included. The data of pre- and postoperative neurological functions, extent of resection, and magnetic resonance imaging (MRI) parameters including preoperative tumor volume and tumor volume reduction (TVR) rate were reviewed. Patients were followed up for 3-64 months (median of 12 months). RESULTS The WHO grade was 1 in 22 cases, 2 in 14 cases, 3 in 12 cases, and 4 in 12 cases. According to the surgical approach, 30 patients underwent unilateral decompression and 40 patients underwent bilateral decompression. There were significant differences in preoperative neurological symptoms between unilateral and bilateral decompression groups. The preoperative maximal tumor diameter, maximal tumor diameter after surgery, TVR rate, and preoperative tumor volume were larger in patients with bilateral decompression than those with unilateral decompression (p < 0.05). Unilateral or bilateral decompression could achieve a total removal rate of 100%, 94.1%, and 93.3% and a TVR rate of 100%, 93.8%, and 96.6%, respectively. The neurological deficits were significantly ameliorated and the MRI showed the tumor had shrunk after surgery.
