Technical aspects of the performance of ultra high resolution computed tomography in the assessment of congenital heart disease.
Ultra-high-resolution CT (UHRC) has increased the accuracy and sensitivity for the diagnosis of congenital heart disease (CHD). The purpose of this study was to evaluate the technical aspects and pitfalls associated with UHRC. Sixteen patients with CHD (7 with tetralogy of Fallot [ToF] and 8 with ventricular septal defect [VSD]) and 2 non-CHD control patients were scanned on a multislice helical CT system. Scan parameters were: collimation 1.5 mm, slice width 0.5 mm, pitch 0.4 mm, rotation time of 1 s. Three concentrations of contrast medium were used: 50, 30, and 20 ml/s, with a delay of 30 s and a duration of 1 min per concentration. Reconstruction included soft-tissue and bone images with a soft-tissue window and a high-contrast sharp reconstruction of bone images. The examination was performed with a mediastinal window to evaluate the status of the chest and the plexus, and a cardiac window to evaluate the status of the heart. Visualization of the great vessels was evaluated by the presence of “open” and “closed” signs. Soft-tissue images were evaluated for the presence of lung parenchymal lesions. High-contrast sharp images of bone were evaluated for bone detail. An experienced radiologist and radiologic technologists, with expertise in UHRC, independently evaluated the images. Concordance between the two observers was evaluated. The examination was technically feasible in 15 of 16 patients.
